We've watched this a few times and we're still not sure exactly what's going on, but it does appear to equal parts fun and stupid. Some sort of electronic guitar pick and belt-mounted speaker lets you play a variety of rockin' tunes, with which to impress the ladies. While it has the Guitar Hero brand on it, it doesn't - as far as we can tell - connect to the game in any way. It's purely for showing off/being a bit of a dick. Entertaining for about, ooh, five minutes, we'd say.
